This disease often occurs in repetitive movements that require extreme abduction of the shoulder joints (such as baseball, freestyle, backstroke and butterfly strokes, weightlifting, and racket sports). When the upper limbs are extended, the humeral head strikes the shoulder and the sacral ligament forward, causing damage to the supraspinatus tendon. Chronic stimulation can cause acromion bursitis, aseptic inflammation and tendon erosion. Acute and intense violence can cause the rotating belt to break. Main symptoms and diagnosis:

For the early stages of the disease, pain only occurs when the upper arm is lifted high and the abduction is forced. Later, when the arm is shaken, it will also appear. Normally, the pain attacks when pushing forward, and there is no or only slight pain when pulling things.

When the upper arm is extended and the shoulder joint is abducted, the rotation is tender, especially when the affected limb is higher than the plane of the shoulder joint (if the affected limb is lower than the body side, there is no tenderness). Excessive chest pain in the affected limb can also cause severe pain. The decrease in abductor muscle strength of the shoulder joint is usually due to the disuse of the deltoid muscle. MRI examination does not suggest partial tearing of the rotating belt, but can show complete tearing.

Pain when pulling upper limbs and shoulders down or holding heavy objects: The most common first symptom of neck ribs is hand and shoulder dull pain, which is intermittent. Pain increases when the upper limbs and shoulders are pulled down, or when heavy objects are held in hand. The neck rib is a congenital deformed rib that can be caused by trauma, shoulder weight factors, and the like.

Pain in the forearm and wrist: a common clinical manifestation of anterior interosseous nerve compression syndrome. Anterior interosseous entrapment nerve syndrome was reported by Kiloh and Nevin in 1952. Subsequently, the relevant cases continued to be reported. Its incidence accounts for about 1% of the distal forearm neuropathy.

Upper extremity abduction is difficult: polymyositis manifests as difficulty in upper extremity abduction. The skeletal muscle pain and atrophy usually start from the proximal end, such as the pelvic belt muscle involvement, and it is difficult to go upstairs; the shoulder swelling is affected by the development of two arms and the difficult lesions can accumulate muscle and neck muscles. Dysphagia and dysarthria occur in the pharyngeal muscles, and a small number of patients may have difficulty breathing. In the acute phase, the muscles often have pain and often have muscle atrophy. Some patients may have arrhythmia and heart block.
