(1) Causes of the disease

Benign acanthosis nigricans is a type of genetic syndrome. Irregular autosomal dominant inheritance, not related to visceral diseases, benign acanthosis nigricans and endocrine diseases: except for hereditary forms, benign acanthosis nigricans can be associated with pituitary, adrenal and ovarian tumors, often in adolescence. Obesity does not necessarily exist.

Malignant acanthosis nigricans: This disease is almost always associated with visceral tumors. About 60% of patients have skin symptoms and visceral malignant progress. About 20% of patients have skin symptoms before malignant tumors for several years. About 20% of patients with malignant tumors are primary, and skin diseases are secondary.

(two) pathogenesis

The pathogenesis is still unclear. Benign acanthosis nigricans, irregular autosomal dominant inheritance, has nothing to do with visceral diseases, malignant acanthosis nigricans, about 60% of patients with skin symptoms and visceral malignant progress, about 20% of patients with skin symptoms before malignant tumors for several years About 20% of patients with malignant tumors are primary, and skin diseases are secondary.

The appearance of various types of acanthosis nigricans is consistent, the severity is different, and the symmetry is the same. The area affected by the skin is the axillary fossa, the flexion of the neck and the napkin area, the vulva, the inner thigh, the face, the elbow fossa, the fossa, Navel, back of the hand, areola, foot, eyelids and nasal vestibule.

The main symptoms are warts, papillary hyperplasia, hyperkeratosis and pigmentation. The initial change was a yellow, gray or brownish yellow color, followed by a deepening of melanin and a clear boundary with the surrounding skin. Velvet papillary-like sacral lesions occur in skin wrinkles and wrinkles, and ash to black hyperplasia often leads to cockscomb-like skin sputum, with varying degrees of hyperkeratosis. Eventually, sickle-like creatures can occur. The affected area can be both limited and extensive, such as the entire armpit. The most severe papillary keratosis hyperplasia is seen in the center of the lesion, tending to reduce the marginal lesions, surrounded by melanin deepening. Impregnation occurs in the inter-finger region, which in turn stimulates proliferation, and in the malignant form, palmar is particularly susceptible. The entire skin is noticeably dry and rough.

Oral mucosal changes are mainly characterized by velvety coat damage on the tongue, but papillary neoplasms can also occur, and can also occur in the lips and tendons. When skin lesions are impregnated, patients with acanthosis nigricans may develop some subjective symptoms and itching. Itching is more common in malignant acanthosis, sometimes very serious. According to the increase of pigmentation in the skin folds, with sputum proliferation, histopathology shows papillary-like hyperplasia, should consider the diagnosis of this disease. The important thing is to distinguish between benign and malignant. Malignant acanthosis skin disease usually occurs in adulthood, severe damage, limbs and mucous membranes are often involved, pigmentation is significant, skin lesions are severe, and itching.

Diagnosis

Differential diagnosis

Itchy skin: Itchy skin refers to a skin disease that has no primary rash but is itchy. It is a neuropsychiatric skin disease and is a cutaneous neurosis disorder. Clinically, only those with itchy skin and no primary skin lesions are called pruritus. It belongs to the category of "itchy wind" of Chinese medicine. Itchy skin is divided into common and allergic types. It can occur all over the body, especially in the face, back and limbs. Ordinary skin itching is generally caused by too dry skin. It can be taken orally with cod liver oil pills, multivitamin tablets, etc. Western medicine must be diagnosed and guided by a professional doctor. Do not blindly use drugs, especially those containing hormones.

Itching is hot when the skin is cold and itching is hot: frostbite is a localized inflammatory damage caused by cold. Frostbite is a common disease in winter, especially for children, women and the elderly. Once frostbite occurs, it is often difficult to cure quickly in the cold season. It will not heal until the weather turns warm. To reduce the occurrence of frostbite, the key is to prevent it before winter.
