Digestive irritation

Introduction

Introduction Digestive tract irritation is also known as irritable bowel syndrome. Refers to a group of clinical syndromes including abdominal pain, bloating, changes in bowel habits, abnormal stool characteristics, mucus, etc., persistent or recurrent, and examined to rule out organic diseases that can cause these symptoms. This disease is the most common type of functional bowel disease. Typical symptoms are abdominal pain and abdominal distension associated with abnormal bowel movements. According to the main symptoms, they are classified into diarrhea-predominant type, constipation-predominant type, and diarrhea constipation alternating type. Factors such as spirit, diet, and cold can induce recurrence or aggravation of symptoms.

Cause

Cause

The cause is not clear and no anatomical reasons can be found. Emotional factors, diet, drugs or hormones can trigger or aggravate this high-tension gastrointestinal movement. Some patients have anxiety disorders; especially phobias, adult depression and physical symptoms. However, stress and emotional distress are not always accompanied by the onset and repetition of symptoms. Some IBS patients exhibit an acquired abnormal pathological behavior. For example, they tend to express mental distress as a complaint of the digestive tract, usually abdominal pain, when the physician evaluates IBS, especially in patients with intractable symptoms. Should understand whether they have unsolvable psychological problems, including sexual abuse and physical abuse.

Diagnostic criteria: The reference standards for clinical diagnosis of IBS developed in China in 1986 are:

1, with abdominal pain, bloating, diarrhea or constipation as the main complaint, accompanied by systemic neurological symptoms (symptoms persist or repeated more than 3 months).

2, the general situation is good, no weight loss and fever, system physical examination only found.

3, multiple fecal routine and culture (at least 3 times) were negative, fecal occult blood test was negative.

4, X-ray barium enema examination without positive findings, or colonic irritation signs.

5, colonoscopy showed that some patients with hyperkinesia, no obvious mucosal abnormalities, histological examination is basically normal.

6, blood, urine routine normal, normal blood sedimentation.

7, no history of parasitic diseases such as dysentery, schistosomiasis, experimental treatment is invalid (Note: nail nitrazole test treatment and discontinuation of dairy products) Those who meet the above criteria can generally make a clinical diagnosis. However, it is necessary to pay attention to the identification of other diseases that are characterized by occult or atypical symptoms. Those who have doubts about the diagnosis may choose to have further examinations.

Diagnosis

Differential diagnosis

Intestinal irritation syndrome is mainly characterized by abdominal pain, bloating, diarrhea and constipation, accompanied by systemic neurological symptoms. The differential diagnosis should be based on which symptom is mainly differentiated from the corresponding disease. For example, abdominal pain should be differentiated from diseases that cause abdominal pain. Patients with diarrhea should be differentiated from diseases that cause diarrhea, where lactose intolerance is common and difficult to identify. The secretive person should be differentiated from the disease causing constipation, in which constipation caused by habitual constipation and adverse drug reactions is common.
