It is mainly determined according to the clinical manifestations. If necessary, the dandruff can be scraped from the edge of the lesion for direct microscopic examination. After the fungal hyphae or spores are found, the diagnosis can be confirmed.

Histopathology: commonly used PAS staining, hyphae and spores can be found in the stratum corneum of the epidermis, and hyphae grow in parallel with the stratum corneum. In the case of granulomatous lesions, hyphae can be found in the hair follicle and upper part of the dermis. The epidermis may have parakeratosis, hyperkeratosis, thickening of the spinous layer, edema of the dermal papilla, infiltration of inflammatory cells around the blood vessels, and sometimes under the stratum corneum. Differential diagnosis of body lice:

1. Rose pityriasis is easily confused with body lice, but it does not occur above the neck. The skin lesions are oval, with marginal scales, and the inflammation is more pronounced. 2. Seborrheic dermatitis occurs in the fatty area, the damage is subacute inflammation, the surface is greasy, small scales, and itching is severe. The fungal test was negative.

3. Psoriasis vulgaris is sometimes ring-shaped, but the base is light red infiltration, covered with multiple layers of silvery white scales, a bit of bleeding after stripping. Occurs in the extremities and joints of the extremities. The fungal test was negative.

In addition, it needs to be differentiated from chronic eczema, neurodermatitis, and simple pityriasis.
