Shrinkage of the surface of the subcutaneous lesions stimulated by cold

Introduction

Introduction Shrinkage on the surface of the skin lesions under cold stimulation is an abnormal proliferation of smooth muscle of the skin. A single or cluster of painful papules or nodules appears in the clinic, and cutaneous leiomyomatosis should be considered when the surface of the lesion is shrunk under cold stimulation. Leiomyoma cutis is a benign tumor composed of skin fascia, mesothelial or vascular smooth muscle. There are few skin smooth muscles, mainly found in the hairy muscles near the hair follicles, the muscle fibers around the deep subcutaneous blood vessels, the smooth muscle bundles in the scrotum and the labia.

Cause

Cause

Abnormal hyperplasia of smooth muscle of the skin. Leiomyoma is composed of smooth muscle cells, which are mostly fusiform or slightly corrugated, often arranged in parallel. The nucleus is central, blunt, cytoplasmic, and contains longitudinal fibrils. Leiomyoma located in the subcutaneous tissue often has an envelope located in the dermis. Although the boundary is clear, it is generally non-enveloped. Angiomyofibroma often has a complete envelope, which contains many large blood vessels that do not contain elastic membranes.

Single or clustered painful papules or nodules appear and shrinkage occurs on the surface of the lesion under cold irritation. When a small piece of ice is placed on the lesion for a few seconds, the smooth muscle shrinks and the surface of the fibroid shrinks, which is one of the diagnostic features.

Diagnosis

Differential diagnosis

The symptoms of wrinkling on the surface of the skin lesions under cold stimulation need to be distinguished from the following symptoms.

Small sweat gland

Small and softly distributed evenly without pain. Genital leiomyomas contract when exposed to cold or touch; single vascular leiomyoma has pain, and multiple or single skin leiomyomas are painful.

2. Dermal fibroma

Mainly for the proliferation of collagen fibers, under the microscope, the cells are slender and lightly stained with red; the ends of the nucleus ellipse are sharp. The collagen fibers were red when Van Gieson was stained and the collagen fibers were green when Masson stained.

3. Schwannomas

It occurs when the nerve trunk is located on the flex side of the limbs and can be pushed up and down, but cannot move up and down. Pathological examination showed that the tumor cells were more slender and wavy. The cytoplasm is stained with light red nucleus and is arranged in a fence.

4. Highly differentiated leiomyosarcoma

Mild atypicality of tumor cells One or several mitotic figures were found every 30 high power fields.
