Reconstruction of thumb-to-palm function with abductor dactylus displacement

Small finger muscle extension, thumb and palm function reconstruction for the treatment of median nerve injury Treatment of diseases: extensor tendon injury preoperative preparation 1. Brachial plexus anesthesia or local anesthesia. 2, supine position, limbs abduction, placed on the table next to the operating table, palm facing up. Surgical procedure 1. Incision: Starting from the ulnar side of the palm of the little finger, along the small fish, the wrist turns to the wrist horizontal stripes, and ends at the midpoint of the wrist transverse stripes. 2, cut the skin, free the flap and then retract to the temporal side to reveal the small fish muscle. The small finger muscles are located in the shallowest layer of the ulnar side, ending at the ulnar side of the proximal phalanx and the extension of the extensor tendon. The small finger muscles are separated and cut off, and are released to the proximal side, retaining the pea bones and the neurovascular bundles that innervate the muscles. Because the neurovascular bundle enters the muscle from the temporal side at the distal end of the lenticular bone, it should be separated from the ulnar side when dissected. 3. Make a longitudinal incision about 3 cm long on the temporal side of the thumb metacarpophalangeal joint to expose the aponeurosis of the buckling short muscles. A subcutaneous tunnel is made between the two incisions through the large fish. Flip the muscles of the little finger and pull out the free end of the thumb through the subcutaneous tunnel. The thumb is placed on the abduction to the palm of the hand, and the tip of the small finger muscle is sutured to the aponeurosis of the abductor muscle. Be careful not to twist or pull the neurovascular bundle that governs the muscles of the little finger.
