little finger abductor transposition

The small finger abductor muscle transposition is used for the reconstruction of the thumb-to-palm function loss. The early thumb-to-palm function reconstruction surgery is not effective. It is mainly because the thumb and palm function reconstruction surgery is not designed to recognize the appropriate power. The fulcrum of the muscle and tension must be at the proximal end of the pea bone. The direction of the pulling force must be applied to the dorsal side of the base of the proximal phalanx of the thumb, and the stop point is selected at the attachment of the short extensor muscle side of the thumb. It is related to techniques such as adjusting the direction of muscle pull. Treatment of diseases: hand flexor tendon injury Indication The small finger abductor muscle transposition is suitable for: 1, large fish muscle paralysis, loss of thumb abduction function. 2. The passive movement of the thumb bone joint is normal. 3. The abductor finger muscle strength is above level 4. Preoperative preparation Brachial anesthesia was chosen. Take the supine position and the upper extremity. Surgical procedure 1. Incision On the surface of the small fish muscle, a convex incision is made to the temporal side, the proximal end is from the wrist transverse line, and the distal end is to the base of the proximal phalanx of the ulnar side of the little finger. 2, reveal and flip the tendon The flap was turned to the ulnar side to reveal the small fish muscle. It can be seen that the abductor muscle is located on the ulnar side of the surface, and the basal part of the proximal phalanx of the little finger and the part of the aponeurosis are continuous with the aponeurosis. Free cut off the aponeurosis and extend as far as possible to avoid length. Retrograde free muscle abdomen until the ulnar half of the pea bone is cut off, and the nutrient vessels and innervated nerves should be protected on the lateral side of the muscle abdomen. 3, tendon fixation A longitudinal incision was made on the temporal side of the metacarpophalangeal joint of the thumb to expose the tendon of the abductor buckling muscle, thereby making an incision, making a subcutaneous tunnel and extending to the first incision of the ulnar side. The small finger abductor muscle was pulled through the subcutaneous tunnel, and the thumb was taken out of the exhibition position, and the tendon was tightened and sutured on the abductor tendon.
