A small number of complete urethral fissures and bladder valgus coexist, called the complex bladder valgus-exospadias complex with pubic symphysis separation. Zhang Fengxiang has proposed a classification method for treatment purposes, which is divided into: 1 incomplete type (penis head and penis type), penis straightening; 2 complete type, penile elongation and anti-urinary incontinence surgery; Complex (with bladder valgus), penile elongation and anti-urinary incontinence surgery and repair of bladder valgus and abdominal wall defects. Any type of male urethral fissure requires surgery. The age of surgery is 4 to 5 years old, the age is too small, and the effect of anti-urinary incontinence is poor. Surgery indications against urinary incontinence should not be too strict, because the bladder neck and posterior urethra reconstruction in addition to anti-urinary incontinence, there is still anti-reverse ejaculation. The urethral fissure without urinary incontinence can be considered for correction of penile malformation and urethroplasty. Bladder neck and posterior urethral reconstruction are the main means of treating urinary incontinence, and there are many methods. Surgical procedure 1. Incision: The midline incision of the lower abdomen reveals the anterior wall of the bladder. 2. Free bladder neck and posterior urethra: the bladder wall and the anterior wall and side wall of the posterior urethra are fully freed in the posterior pubic space, and the urogenital tract is directly reached. 3. Cutting the bladder neck: longitudinally incision of the anterior wall of the bladder, incision in the direction of the urethra to the far side of the plane of the ureteral orifice, and then transversely cut to the bladder wall on both sides, and removed from the lateral wall of the bladder to the lateral edge of the triangle The bladder neck, the anterior wall of the posterior urethra and a part of the side wall, the posterior wall of the bladder neck and the posterior urethra become a smooth muscle strip. 4. Reconstruction of the bladder neck and posterior urethra: The smooth muscle strip formed after cutting is surrounded by a 12-14F catheter, and the muscle strip is continuously or intermittently sutured into a tubular shape with a 3-0 absorbable line. 5.
