Mathieu

Mathieu was designed by Mathieu in 1932 and is also known as urethral basement vascular flap urethroplasty. Treatment of diseases: hypospadias Indication Mathieu is suitable for coronary sulcus type, coronal sulcus type and lower curvature. The urethral opening is located in the first third of the penis. Preoperative preparation 1. If the penis is too small, the male hormone therapy should be applied appropriately. After the penis is developed, the operation is performed again. 2. Prophylactic use of broad-spectrum antibiotics 1d before surgery and continued until wound healing. 3. Wash the skin of the surgical field with a small irritating soap solution. Do not use any cleaning agent that stains the skin and confuses the blood vessels. 4. After anesthesia, separate the foreskin adhesion, expose the coronary sulcus, remove the accumulated smegma, and flush the urethra with 0.5% chlorhexidine or dilute iodine to eliminate possible bacteria. Surgical procedure 1. The 4th thread runs through the penis head for traction, and the artificial penile erection test is performed as appropriate to determine whether there is a penile curvature. 2. The incision is cut into the skin according to the dotted line. The width of the penis flap under the urethral orifice is not <0.5cm, the depth is shallow to the white membrane, and the length is reversed by the flap to the top of the penis head. Be careful not to damage the feeding vessels when cutting the penis flap. The penile head tissue on both sides of the urethral plate incision is separated from the penis white membrane to reach the end of the corpus cavernosum, so that the ventral wing of the penis can be loosely wrapped around the new urethra; and the penis skin sleeve is separated into the root of the penis. 3. Urethral and penile head formation According to the size of the urethra, the corresponding porous silicone tube is placed into the urine drainage and the urethral stent is formed. The urethral basement flap is turned upside down around the silicone tube, and the 5-0 absorbable line and the urethral plate margin are used. Stitched into a new urethra. The wings of the penis head are wrapped around the new urethra, and the two layers are sutured to repair the penis head. 4. Repair the dry wound of the penis and cut the excess foreskin on the dorsal side, and transfer it to the ventral side to wrap the penis to suture.
