Repair of old patellar ligament with fascia lata

The fascia lata repairs the old patellar ligament for the repair of old patellar ligament rupture. The old patellar ligament is broken. Because the retracted quadriceps muscle has fibrosis, direct suturing is difficult. Other methods must be used to repair the defect and reconstruct the tendon. Treatment of diseases: fracture of the tibia Indication The fascia lata repairing old patellar ligament is suitable for: 1. The patellar ligament is completely broken. 2. Some or most of the patellar ligaments with old knee instability. Contraindications A comminuted fracture of the tibia is not healed. Preoperative preparation 1. Conventional knee X-ray examination to understand bone and joint damage. 2. Preoperative iliac bone traction. That is, in the upper part of the humerus, the needle is crossed across the Kirschner wire, and the traction bow is continuously pulled (weight 2 to 3 kg). After the quadriceps muscle is sufficiently extended, the repair is performed. If the traction needle is not infected, it can be removed after surgery. 3. The skin on the outside of the lateral femoral head must be prepared before surgery to cut the fascia. Surgical procedure Incision The anterior U-shaped incision was used to cut the skin and subcutaneous tissue. 2. Reveal the humerus and ligament Sharp separation under deep fascia reveals the surface of the tibia, patellar ligament and tibial tuberosity. 3. Repair the tendon Excision of all scar tissue at the end of the fracture, free patellofemoral ligament and appropriate dressing. A diameter of 0.6 cm bone tunnel was drilled in 1/3 of the tibia. Use the curved forceps or the traction Kirschner wire to pull the humerus down to reduce the patellofemoral gap. Then take a 20cm long fascia strip from the healthy thigh, and pass through the bone tunnel of the humerus to tighten the fascia strip. The suture was fixed on the distal end of the patellar ligament with thick silk thread, and the excess fascia strip was woven and reinforced, the defect was repaired, and the patellar ligament was reconstructed. 4. Suture incision Relax the tourniquet, carefully stop the bleeding, flush the incision with isotonic saline, and suture the incision in layers.
