Stubborn obesity

Introduction

Introduction Through drug treatment, there is still no significant decrease in body weight. Some people advocate 127Kg as the boundary, more than this is intractable obesity. The main causes of intractable obesity are: mental stimulation; unpleasant life; or some human factors promote polyphagia; some serious endocrine diseases, such as Cushing's syndrome, pituitary tumors, etc.; combined with other serious diseases.

Reasons for refractory obesity:

The main causes of intractable obesity are: mental stimulation; unpleasant life; or some human factors promote polyphagia; some serious side effects of endocrine disease drugs, such as by adrenocortical hormone drugs. Phenomenon: Obesity that occurs after a period of medication, such as some patients with allergic diseases, rheumatoid diseases, and asthma. Such as Cushing's syndrome, pituitary tumors, etc.; combined with other serious diseases.

Diagnosis of refractory obesity:

Through diet and medication, there was no significant decrease in body weight.

1. Body mass index (BMI) determination: male 27, female 26.

2. Standard body weight measurement: The measured body weight exceeds 20% of the standard body weight.

3. Determination of fat percentage: percentage of fat > 30%.

Diagnosis

Differential diagnosis

Differential diagnosis of refractory obesity:

Simple obesity

Simple obesity is divided into two types:

Causes of constitutional obesity: Congenital. The metabolism of substances in the body is slow, and the rate of synthesis of matter is greater than the rate of decomposition. Phenomenon: Fat cells are large and numerous, all over the body.

Causes of acquired obesity: caused by overeating. Sweet food in food, greasy food. Fat is distributed on the trunk.

2. Pathological obesity

Causes of Cushing's syndrome: Adrenal hyperfunction, excessive secretion of cortisol, phenomenon: face, neck and body hypertrophy, but the limbs are not much fat.

Pancreatic causes: Excessive insulin secretion, decreased metabolic rate, reduced fat breakdown and increased synthesis. Phenomenon: Whole body obesity.

Causes of decreased sexual function: Cerebral obesity, accompanied by loss of sexual function, or loss of libido. Phenomenon: Breast, lower abdomen, obesity near the genitals.

Pituitary causes: Pituitary lesions cause excessive growth hormone in the anterior pituitary: body bones, soft tissue, visceral hyperplasia and hypertrophy.

Causes of hypothyroidism: Hypothyroidism. Phenomenon: Obesity and mucous edema.

Drug-induced causes: The side effects of the drug are caused, for example, by adrenocortical hormone drugs. Phenomenon: Obesity that occurs after a period of medication, such as some patients with allergic diseases, rheumatoid diseases, and asthma.
