Vascular Repair
The principle of treatment of major arterial injury is mainly to restore the continuity of the artery. The sooner the injured artery is repaired, the better. It is best to perform it within 6-8 hours after the injury. Delays in time will greatly increase the chance of distant intravascular thrombosis or infection, thereby reducing the likelihood of recovery from blood supply. However, if the distal blood vessels are not smooth, even if the time is longer, even more than 20 hours, the repair surgery is still possible. Treatment of diseases: vascular injury, hand vascular injury, limb vascular injury Indication The penetrating injury caused by sharp sharps, the wounds are clean, the vascular clefts are small and neat, and it is estimated that the postoperative lumens will not be significantly narrowed before they can be repaired. If the wound is heavily contaminated, or if the blood vessels and surrounding tissues are squeezed, it is not advisable to perform a simple repair. Surgical procedure 1. Rinse the end of the lumen: After blocking the blood flow at both ends of the blood vessel with a pair of non-invasive blood vessel clamps, use a smooth plastic tube or a blunt needle to extend into the vascular cleft to suck the hemorrhage block, then rinse with heparin solution. . 2. Exfoliation of the extravascular membrane: The outer membrane of the blood vessel around the rupture was peeled off by a small scissors to a length of about 1 cm. 3. Close the split edge: the suture should preferably be perpendicular or close to the longitudinal axis of the blood vessel to avoid stenosis. The blood vessels at both ends of the blood vessel are pulled in the middle to bring the two edges of the blood vessel slit closer together. 4. Stitching slit: The filament thread lubricated with liquid paraffin, the needle is placed 0.5 to 1 mm away from the edge of the slit, and the stitch length is also 0.5 to 1 mm, and the intermittent simple or intermittent valgus suture is performed. If the rupture is large, it can also take a section of the autologous vein, and cut into a vein to fill the rupture of the vein to avoid stenosis caused by direct suturing.
