Intra-atrial diversion surgery

Use a pericardium or polyester fabric to build a barrier in the right atrium, placed around the superior and inferior vena cava, and direct the blood of the vena cava (ie, the venous blood of the systemic circulation) to the tricuspid valve and into the left ventricle into the lung. Pulmonary venous blood is directed to the tricuspid valve and into the anterior right ventricle into the aorta. Although the anatomy makes the deformity more complicated, it has a physiological function in hemodynamics. Treatment of diseases: transposition of pediatric aorta Indication 1. Complete ventricular septal transposition of the great aorta. 2. The ratio of left ventricle to right ventricle pressure is less than 0.6. Preoperative preparation 1. Conventional preparation of cardiopulmonary bypass surgery. 2. Oxygen inhalation, cardiac diuretic treatment of heart failure, correction of acidosis. Surgical procedure 1. The sternal midline incision. 2. Cut the pericardium. 3. Establish extracorporeal circulation. 4. For the right atrial longitudinal incision. 5. Exposure of atrial septal defect and pulmonary veins. 6. Expand the atrial septal defect. 7. The pericardial patch is sutured along the left edge of the pulmonary vein opening around the posterior wall of the left and right atrium between the upper and lower pulmonary vein openings and the superior and inferior vena cava openings. 8. Lift the pericardial piece and trim it. 9. The trimmed pericardial patch is sutured around the superior and inferior vena cava to the atrial septal stump, and an intracardiac passage is established between the superior and inferior vena cava and the mitral valve.
