Ilium Rotation Osteotomy and Bone Graft Lengthening

The iliac bone rotation and bone grafting is used for the treatment of hip joint deformity. The tibial rotation osteotomy and bone graft extension is suitable for hip dislocation and short limb deformity, and there is no deformity at the upper end of the femur. Contraindications 1. Poor general condition and infection of the skin in the surgical area. 2. The femoral head does not reach the position of the acetabulum relative to the plane. 3. The head is large and small, and the joint surface is inconsistent. 4. Hip joint mobility is significantly limited. 5. Being too old. Preoperative preparation 1. Do a good job in the general condition and skin preparation in the operating area. 2. Preparing blood: If it is estimated that the operation is difficult or needs to be done at the same time, it should be matched with blood 300-600ml. 3. The bladder must be emptied or indwelled before surgery. Surgical procedure Incision Starting from the middle of the iliac crest, the skin and subcutaneous tissue were cut along the iliac crest to 4 to 5 cm below the anterior inferior iliac spine. 2. Osteotomy The diaphragm is attached to the inner and outer humerus of the humerus with a periosteal stripper. The anterior and posterior periosteum of the superior iliac crest is peeled off to the deep part of the sciatic bone, and the large ischial notch is obtained. The gauze is used to fill the large ischial notch. The right angle vascular forceps are used to extend from the inside of the large hole of the ischial bone, and the wire saw is clamped. After being pulled out from the outside, the tibia is cut off between the upper and lower spines. 3. Bone graft extension Use the spreader to gradually open it, fix it with four-hole steel plate screws, insert the trapezoidal bone block into the bone graft from the sputum, and rotate the distal humerus forward and downward to make the acetabulum cover the femoral head.
