External oblique muscle replacement for gluteus medius
Extra-abdominal oblique muscle gluteus medius is used for the treatment of hip sac deformity, which is a muscle replacement of gluteal muscle paralysis. Treatment of diseases: congenital hip abduction contracture and pelvic tilt Indication Abdominal external oblique gluteus medius is suitable for hip joint instability caused by gluteal muscle paralysis, but the abdominal external oblique muscle strength is above grade 4. Contraindications Patients with coagulopathy are treated after surgery. Preoperative preparation Prepare routinely before surgery. Surgical procedure Incision The skin and subcutaneous tissue were cut from the posterior iliac crest line to the pubic tuberosity. 2. Free external oblique muscle Two incisions parallel to the inguinal ligament were made on the aponeurosis of the external oblique muscle. The aponeurosis was cut into 2 cm wide aponeurosis strips. The proximal end of the incision was stopped at the proximal side of the pubic tuberosity 2 cm along the lower edge of the external oblique muscle. Below the rib arch, the free external oblique muscle and the aponeurosis are sutured into a tubular shape, and the sacral nodule is repaired upward from the pubic tubercle. 3. Fixed muscle stop A longitudinal incision was made on the outside of the iliac crest to reveal a large trochanter, and a 1 cm diameter bone hole was drilled in the large trochanter. A subcutaneous tunnel is made between the incisions, and the free end of the external oblique muscle is pulled into the second incision through the subcutaneous tunnel. When the diaphragm is too short, the fascia can be prolonged. In the mild outbone position of the hip, the external oblique muscle tendon is passed through. The bone hole on the trochanter is fixed by braided suture.
