Dwyer osteotomy

Dwyer osteotomy is used for surgical treatment of varus deformities. Treatment of diseases: congenital clubfoot Indication Dwyer osteotomy is used to form a fixed varus deformity in the heel. Preoperative preparation Regular preoperative examination. Surgical procedure 1. A curved incision is made parallel to the tendon 1 cm below the long iliac crest to expose the lateral side of the foot. The upper edge of the incision was retracted, the tendon sheath of the humerus longus was exposed, and the periosteum of the calcaneus was removed from the upper side, the side and the lower side at the rear. 2. A wedge-shaped osteotomy parallel to the tendon is placed immediately below and behind the long iliac crest to remove the wedge. The base of the bone wedge is about 8 to 12 mm wide depending on the need for orthopedics. The bone wedge should reach the medial cortex of the calcaneus but not penetrate. The technique is used to fracture the medial cortex and close the wedge-shaped osteotomy space; further, the dorsiflexion of the foot against the tension of the Achilles tendon makes the osteotomy area close. If the closure fails, the tip of the wedge is still present and should be removed. Confirm that the varus deformity has been corrected and the incision is closed after the heel is in the neutral position or mild inversion.
