arterial auscultation
The arterial auscultation examination is an auxiliary examination method for checking whether the blood vessel pulse is normal. If the vascular lesion is suspected, it can be selectively auscultated at the carotid artery (the inner edge of the sternocleidomastoid muscle, the height of the upper edge of the thyroid cartilage), the subclavian artery, the renal artery, the abdominal aorta, the radial artery, and the femoral artery. Under normal circumstances, only two heart sounds consistent with S1 and S2 can be heard on the carotid artery and the supraclavicular artery, which is called normal arterial sound. Arterial murmurs are more common in peripheral arteries and in the pulmonary arteries and coronary arteries. Through this examination, the lesion and the corresponding symptoms can be judged. Basic Information Specialist Category: Cardiovascular Examination Category: Other Examinations Applicable gender: whether men and women apply fasting: not fasting Tips: Maintain a normal diet and schedule. Normal value Under normal circumstances, only two heart sounds consistent with S1 and S2 can be heard on the carotid artery and the supraclavicular artery, which is called normal arterial sound. Normal young people can also hear a systolic murmur in the abdominal aorta, so they need to be combined with other tests to make a comprehensive judgment. Clinical significance Abnormal result 1. Neck vascular murmur. Carotid or vertebral artery stenosis should be considered if vascular murmurs are heard in the large vessel area of the neck. The murmur is generally marked by arteritis or atherosclerosis during systole. 2. Abdominal vascular murmur. Normal young people can also hear a systolic murmur in the abdominal aorta, so they need to be combined with other tests to make a comprehensive judgment. Hypertensive patients should be aware of the possibility of renal artery stenosis when they hear systolic murmurs in the mid-upper and lower flank. 3. Shooting sounds. When the aortic valve is insufficiency, the chest piece of the stethoscope is placed in the femoral artery, and the sound of a pistol shot that matches the systole of the heart can be heard at the radial artery. It is because the pulse pressure difference increases, the pulse impacts the arterial wall, and other diseases that cause an increase in pulse pressure difference, such as patent ductus arteriosus, hyperthyroidism, etc., can also be heard. 4. Supraclavicular arterial murmur (supraclaviculararterialmurmur) is also known as "neck arterial murmur". The supraclavicular artery murmur is a physiological murmur. Seen in the state of high cardiac output, especially in anemia and anxiety. 5. Left upper abdomen vascular murmur (leftupperabdomenvesselmurmur) is caused by the compression of the celiac artery by the pancreatic body and tail cancer, especially the compression of the splenic artery. It is considered to be an important characteristic feature of the early stage of pancreatic body and tail cancer. Splenic aneurysms, which are rare in clinical practice, can also produce systolic murmurs in the left upper abdomen, often identified by means of angiography. 6. Liver area arterial murmur (liverareaarterialmurmur), liver area arterial murmur is caused by liver cancer compression of the hepatic artery or abdominal aorta, but also from the blood vessels of the cancer itself or the disease of the hepatic artery itself. Therefore, it is considered to have a certain diagnostic value for primary liver cancer. In addition, liver abscess, polycystic liver, secondary liver cancer, etc. can also produce this noise. 7. Costospinalanglevesselmurmur refers to this murmur is more important for the diagnosis of renal artery stenosis. The vascular murmur of renal artery stenosis can be heard at about 7 cm above the umbilicus of the abdomen or 2.5 to 3.5 cm from the midline to the affected side. The clinical significance is the same as that of the rib horn. People who need to be examined: routine physical examination items, abnormal blood vessels. Precautions Forbidden before examination: Maintain a normal diet and schedule. Requirements for examination: The patient actively cooperates with the doctor during the examination. It is not suitable for strenuous exercise one hour before the examination. It is not appropriate to be overly stressed during the examination, so as not to affect the result. Inspection process The contents and methods of the inspection are as follows: 1. The arterial gunshot sound (arterialpistolshotsound) produces a loud "嗒-嗒-" sound when the arterial pulse is heard at the radial artery and femoral artery, which is a long, similar gunshot sound, hence the name. It is a peripheral vascular sign of aortic regurgitation. As the pulse pressure increases, the arteries are highly dynamic and pulsate, causing the pulse wave to impact the arterial wall. If you have a little pressure, you can also hear the double murmur of arterial systolic and diastolic, called Duroziez double murmur. In addition to aortic regurgitation, it can also be seen in patients with hyperthermia, hyperthyroidism, anemia, patent ductus arteriosus, main-pulmonary septal defect, and coronary artery spasm. Occasionally seen in individual hypertension, arteriosclerosis patients. 2. The supraclavicular arterial murmur (supraclaviculararterial murmur) is also called "neck arterial murmur". Refers to a soft, short, tremor-free, non-receptor, and respiratory-like murmur that is heard on both sides of the collarbone (clearly on the right). For example, if the chest piece of the stethoscope is gently pressed against the first rib at the clavicle, the noise will change, and it will disappear when the pressure is applied. 3. The left upper abdomen vascular murmur (leftupperabdomenvesselmurmur) in the left upper abdomen smells a short systolic murmur, more limited, generally not far. 4. Liver area arterial murmur (liverareaarterialmurmur) refers to a high-profile continuous murmur or continuous murmur late reinforcement in the upper abdomen or right anomalously swollen liver. 5. Costospinalanglevesselmurmur refers to a continuous or systolic murmur with a low loudness, high pitch, and long duration that is heard at the left or right rib angle. Not suitable for the crowd Inappropriate people: severe blood vessels and heart failure. Adverse reactions and risks Nothing.
