Intravenous pyelography (IVP)
A venous angiography of the renal pelvis was performed by X-ray examination. Intravenous pyelography is an intravenous injection of contrast agent, the lower abdomen is pressurized, and the film is examined 10 to 20 minutes later. Intravenous pyelography can understand the relationship between the kidney, the location of the ureter, the secretory function of the kidney, and the retroperitoneal lesions and the urinary organs. Kidney, ureter and bladder tuberculosis, tumors, etc. Unexplained hematuria. Urinary calculi, determine the location of the stone, and see if there are negative stones. Basic Information Specialist classification: urinary examination classification: X-ray Applicable gender: whether men and women apply fasting: fasting Tips: You must take laxatives before going to bed the night before you check. Do not drink water and enter the liquid food for three hours before the angiography (you can eat a small amount of dry food such as flatbread, bread, etc.). Please bring the contrast agent to the radiology department when you come to check. If there is a special situation on the day of the inspection, please contact us two days in advance, otherwise it will expire. Normal value Indications 1. Kidney, ureter and bladder tuberculosis, tumors, etc. 2. Unexplained hematuria. 3. Urinary calculi, determine the location of the stone, and understand whether there are negative stones. Clinical significance Intravenous pyelography can understand the location of the kidney, ureter, the secretory function of the kidney, the relationship between retroperitoneal lesions and urinary organs. Precautions 1. Before taking the examination, take a plain film of the abdomen for reference. If kidney function damage is suspected, renal function should be measured first. 2. Check the pre-dinner for half-flow food, and fast after dinner until the angiography. 3. Check the laxative before the night or the enema before the examination, and check according to the agreed time. First look at the abdomen and observe if there is any gas or fecal mass in the bowel for further treatment. 4. Iodine allergy test. Drain the urine. 5. Commonly used contrast agent 60% diatrizoate. Dosage: 4 to 6 ml below 1 year old, 5 to 10 ml from 2 to 6 years old, 10 to 15 ml from 7 to 14 years old, and 20 ml for adults over 15 years old. 6. When checking: Actively cooperate with the doctor. After 3 days of inspection, the outpatients should go to the outpatient service desk to get a report based on the medical history card. Not suitable for the crowd: In order to avoid the radiation effects of X-ray on the fetus, pregnant women should try not to do intravenous pyelography. Acute infectious diseases, high fever, severe cardiovascular and liver diseases, acute urinary tract infections, pregnancy and puerperium are not suitable for this examination. Inspection process 1. The patient bends his knees and lays the abdominal pressure belt. 2. Slowly inject the contrast agent into the forearm vein. It should be stopped after 1ml injection. If there is no discomfort after 5min, it can be injected within 4~5min. 3. Immediately after the injection, the abdominal pressure is applied (the lesion must be cautious when there is a lesion in the abdomen, and the pressure is added so as not to interfere with the breathing and not to cause the patient to feel pain). According to the need, take one tablet at 7min and 15min (including the kidney areas on both sides), and take a full abdominal piece after 30min decompression. You can also adjust the shooting time under TV perspective. Children should take a little filming time, and they can take pictures at 3, 7 and 15 minutes after the injection. 4. High-dose method, suitable for patients with poor renal function or suspected ureteral disease, can be calculated with contrast agent 1.5 ~ 2ml / kg, add the same amount of 5% glucose solution or normal saline, rapid intravenous input, should be input 200ml in the first 6min. After the loss, 1, 3, 5, 10, 20 minutes each took one. This method does not need to be water-free, and does not require abdominal pressure. 5. In case of special circumstances, the filming time and location can be arranged separately by the radiologist. 6. If you need to check the bladder, you can decompress it and wait for it. When the bladder is full, take a positive position and take the left and right oblique slices if necessary. Not suitable for the crowd Iodine allergy, severe liver or kidney dysfunction, acute nephritis, acute pyelonephritis, severe heart disease, heart failure, hyperthyroidism, cachexia. Adverse reactions and risks May cause an infection.
